Responsibilities
- Provide direct services to children, youth, young adults, and their families
- Collaboratively plan services with child and family teams
- Coordinate supports and services across community and clinical providers
- Monitor care plans to ensure access to medically necessary services
- Maintain required documentation and progress notes
Qualifications
- Bachelor’s Degree in a Social Science or Behavioral Health field required
- Valid Alabama driver’s license required
