Job Description

PRIMARY JOB FUNCTIONS AND PERFORMANCE EXPECTATIONS:

1. Coordinate Patient Assistance for medications.
a) Complete forms, gather financial information and obtain patient signatures.
b) Take delivery of patient assistance meds – log in and store.
c) Obtain patient signature when meds are picked up.
d) Scan all paperwork into electronic health record.

2. Monitor prescription rewrite request.
a) Access prescription need and enter request into electronic health record.
b) Serve as contact person for consumers regarding medication needs

3. Provide support with prior authorizations and overrides.
a) Assist with coordination of prior authorizations/override forms as completed by nurses
and obtain MD/CRNP signature if needed.
b) Fax to insurance company and pharmacy and scan document into electronic health
record.

4. Assist with other consumer medication needs.
a) Order and distribute injectable medications as needed.
b) Assist with medication samples as directed by medical providers.
c) Coordinate with Executive Director if medication payment assistance is needed for
consumers.
d) Assist with coordinating lab work as needed as directed by medical providers.

5. Perform other duties as needed, including assisting in other administrative roles.
a) Complete consumer documentation to schedule initial and follow-up appointments,
complete intake and annual documentation, etc.

Prerequisites

Requires a High School Diploma, proficient computer skills, and knowledge of psychotropic meds preferred
