In Home Youth and Family Co-Therapist/Case Manager, Selma

Job Description

1. Provide direct services to assigned caseload for In-Home Services.
a) Responsible for management of client medications.
b) Conduct face-to-face interviews with clients and significant others to assist with accomplishment of service plan.
c) Identify appropriate community service needs of those persons in need of case management and determine appropriate community resources to
meet those needs.
d) Advocate on behalf of the clients with regard to social, economic and residential requirements.
e) Assisting the client through crisis situations and/or arranging for the provision of such assistance by other professional/personal care givers.
f) Completes Human Service Needs Assessment as required.

2. Administers injectable medications and supervises oral medications for assigned caseload.
a) Administers injectable medications as prescribed in a timely manner.
b) Checks clients for treatment response and possible side effects.
c) Check for medication compliance.
d) Check blood pressure and weight of all clients receiving injectables and other clients as needed.
e) Serve as back-up nurse for Group Homes and injection clinic as needed.

3. Maintain client’s files and related paperwork of acceptable quality in a timely manner.
a) Develop written service plans for each client and evaluate and revise plan as needed.
b) Complete progress notes and billing sheets as required.
c) Record medication on medication form.
d) Completes Medication Administration/Medication Monitoring Form when giving injections.
e) Record any significant behavioral observations in client’s record and notify physician and/or client’s therapist when appropriate.
f) Schedule client’s return appointments.

4. Provide transportation to clients.
a) Transport only clients who actually need transportation.
b) Ensure that all community resources, such as family, friends or public transportation are utilized.
c) Maintain a valid Alabama Driver’s License.

5. Other duties as assigned.
a) Attends and actively participates in In-service and Quality Review as required.
b) Utilizes supervision/consultation by Medical Director as indicated.
c) Attend staff meetings as required.
d) Serves on Pharmacy Committee.
e) Maintains professional license.
f) Available to LPN’s for supervision as needed.
g) Serve as on-call nurse for residential programs.

Prerequisites

Requires a Bachelor's Degree in Human Services related field.
Also, a valid AL driver's license.
