Developmental Specialist, Early Intervention Program
Job Description
1. Coordinate/monitor the delivery of available services with the individual/family.
a) Receive referrals of developmentally delayed children residing in catchment area.
b) Assist family in access of services by completing appropriate releases and making referrals
as indicated.
c) Conduct family interviews to complete admission paperwork, assess individual/family
assets, needs, secure physician and/or Patient 1st referrals, and schedule required
evaluations.
d) Determine individuals eligibility for targeted case management.
e) Monitor closely and document in the case management records the delivery of services,
outcomes, and the need for further services.
2. Participate in the development and implementation of Individualized Family Service Plans
(IFSP) based on client need.
a) Review intake materials, medical histories, developmental histories, evaluations and other
case summaries on each individual client.
b) Provide input into client goals and objectives by conferring with family to identify family-
defined goals.
c) Write the IFSP in conjunction with family and other professionals in accordance with Part
C standards.
d) Initiate the review and evaluation of the IFSP within six months of implementation and as
often as is otherwise indicated.
e) Notify family in writing of any proposed changes in the IFSP.
3. Develop and implement a transition plan for each child.
a) Discuss transition with family at initial IFSP meeting, 6 months prior to third birthday, and
as otherwise needed to develop transition plan and educate families on other resources.
b) Obtain releases from family to send information to the appropriate receiving agency.
c) Make referral to appropriate receiving agency 6 months before third birthday.
d) Schedule transition meeting between Early Intervention Program, family and receiving
agency to exchange information packet and discuss related issues.
e) Review and revise transition plan as indicated.
4. Complete required reports and other administrative items.
a) Complete and submit all data and reports in accurate, neat and timely manner according to
program standards and Personnel Policies.
b) Insure that all pertinent data in included in client’s record and meets required DMH/ID,
Part C and Medicaid standards.
5. Performs other related duties as necessary or required.
a) Attends and actively participates in meetings.
b) Utilizes supervision/consultation as indicated
Prerequisites
Requires a bachelor's degree in a Human Services field and a valid AL driver's license
